A letter of authorization. The letter of authorization must be on letterhead from the employer or the worker’s comp
insurance. The letter must state that the patient was injured on the job and that they will be covered under worker’s comp
insurance. The authorization letter must include the following:
- Name and address of worker’s comp insurance
- Telephone number of worker’s comp insurance
- Claim number
- Part(s) of the body to be treated
- Date of injury
- Name of work comp adjuster (if available)
Authorization letters for worker’s comp injury may be faxed to us.

X-ray copy. Please contact us at least 2 working days BEFORE you need the X-ray. Also, take a moment to read our
HIPAA policy regarding the confidentiality and release of your private medical records.

medication please call us BEFORE you run out as it can take time to get your new medication.
Co-pays are expected at time of service unless other arrangements have been made in advance. Please know that the patient
is ultimately responsible for all charges incurred and those not covered by insurance.
