What are your office hours?

Mendelson Orthopedics, in St Mary’s
Monday – Friday 8:30am to 5pm

Macomb Orthopedics, in St John
Monday – Thursday 8am to 6pm
Friday 8am to 4:30pm

What do I need to bring to my appointment?

  1. Complete paperwork (download the forms you need on our forms page)
  2. Insurance card(s)
  3. Picture ID (ex drivers license…)
  4. Any recent X-rays (both the films and the associated written reports)
  5. Any recent MRIs (both the films and the associated written reports)
  6. Any other reports from testing pertinent to your visit with the doctor
  7. A list of medications you are currently taking

How do I contact your office?

What do I need if this is a workers compensation case?

A letter of authorization. The letter of authorization must be on letterhead from the employer or the worker’s comp insurance. The letter must state that the patient was injured on the job and that they will be covered under worker’s comp insurance. The authorization letter must include the following:
  1. Name and address of worker’s comp insurance
  2. Telephone number of worker’s comp insurance
  3. Claim number
  4. Part(s) of the body to be treated
  5. Date of injury
  6. Name of work comp adjuster (if available)

What if I need to cancel my appointment or if I’m running late?

Call us. We will make every effort to still see you or reschedule you if need be, but we may need to cancel your appointment.

If you need to cancel please give us as much notice as possible (24 hours minimum). Please go to our locations page and contact us directly.

How do I get a copy of my X-rays or records from you?

Call us. Go to our locations page and contact us directly. There is a charge for the X-ray copy. Please contact us at least 2 working days BEFORE you need the X-ray. Also, take a moment to read our HIPAA policy regarding the confidentiality and release of your private medical records.

How do I get my prescription refilled?

Call us. Because of the medications we prescribe physician approval is required. To prevent running out of your medication please call us BEFORE you run out as it can take time to get your new medication.

When are co-pays due?

Co-pays are expected at time of service unless other arrangements have been made in advance. Please know that the patient is ultimately responsible for all charges incurred and those not covered by insurance.
