What insurances do you accept? (Mendelson Orthopedics)
NOTE: Coverage varies by location. The answer below applies only to Mendelson Orthopedics off Levan Rd.
Coverage also varies by physician, however, below is a partial list of insurances we accept. We do not accept any HMO's
except Aetna and BCN. If you have a different HMO, please contact your primary care doctor directly. Please know that the
patient is ultimately responsible for charges incurred and those not covered by insurance. You should contact your
insurance company to make sure the doctor participates with your individual plan/group.

Does my insurance cover Physical Therapy?
To be certain your insurance covers physical therapy you should call the phone number on the back of your card. Please let
us know if you have received physical therapy any where else within the last year as this can affect payment and coverage.

What if my injury is related to an auto accident or a workmans comp case?
If your case relates to an auto accident or workmans comp case please let us know in advance. Also please review what you
need to bring with you in these cases by going to our Questions for the doctors office section.
When are co-pays do? (Doctors Office)
Co-pays are expected at time of service unless other arrangements have been made in advance. Please know that the patient is ultimately
responsible for all charges incurred and those not covered by insurance.
When are co-pays due? (Physical Therapy)
Because physical therapy is not like a doctors appointment we will generally send you a bill for your co-pays. Coverage can
vary greatly by insurance. Please contact your insurance company to verify coverage and answers to co-pay questions. Please
let us know if you have received physical therapy at another location within the last year as this can affect coverage. Please note the
patient is ultimately responsible for all charges incurred and those not covered by insurance.
